In this model, patients can expect thorough examination, shared decision-making, and building a strong therapeutic alliance. Instead of treating issues in isolation, this model of care empowers the patient to be actively involved in continued self-management.   <\/br><\/br>\n\nIn the current healthcare environment where volume-based treatment is incentivized, we strive to provide outcome-based treatment. I climb 3-4 times a week. This has taken a toll on my shoulder muscles. I could not climb difficult routes due to pain and stiffness in my shoulders and my overall functional movements declined.
